Mode of referral: Through the Specialist Health Visitors, community nurses, Social services, the Child Development Team, Surestart workers and families can self refer.
Eligibility criteria: A family with a child aged 0 to 5 years with a disability/ special need living in the London Borough of Ealing.
Current availability: Places available immediately.
What can I expect to happen?: As soon the name of the family is passed to the Outreach Worker along with some basic details then the Outreach Worker will contact the family and will bring them to P.E.S.T.S on their first visit.
