| Leaving school and moving into adulthood can be a very stressful
time for young people and their parents. Most schools should provide
careers information in a way that is accessible to all pupils. There
is help available and some useful sites are detailed later in this
section.
As far as Education is concerned the SEN Code of Practice says that
transition planning must include:
- What a young person needs to become more independent and confident
- What practical adaptations may be needed at home?
- What special health needs the young person has which require
on-going support
- What the young person and family want to happen
In Year 9 when the young person is 14 the head teacher or LEA
should arrange a review to create a Transition
Plan.
Skill (National Bureau for Students with Disabilities):
Skill gives information and advice to disabled students on education,
volunteering, training, and employment.

JobcentrePlus
Offers help to enable disabled people to get a job. They employ
Disability Employment Advisers who know a lot about the schemes,
which can support disabled people to get a job, and how to get money
towards equipment etc. Jobcentre Plus staff can also tell you how
getting a job may affect your benefits.

The Prince's Trust
Offer practical support, training, mentoring and financial assistance
to young people aged 14-30, to help them succeed and transform their
lives. The types of programmes they offer include working on a community
project; a 6 day residential music course; financial support to
get into work, training or education; and support to set up your
own business.

Education Maintenance Allowance
From September 2004 students who stay on in education after Year 11 (post-16) will be able to claim Education Maintenance Allowance. This is a weekly payment made to the student to help towards the costs of continuing education such as books, transport, equipment etc… The level of allowance granted to a student is dependent upon the family income, but any allowance awarded does not affect the family or young person’s benefits.
